I've got a ball peen hammer of my dad's that close to 70 years old. It's marked 32oz Made in USA. I'd like to find someone that would forge this into a belt style hawk. Any names come to mind?

There are a lot of hawks for sale for quite reasonable prices. Buy two or 3 and get used to your favorite.
Hang the hammer over your bench where you can appreciate it.
Amazon has a nice looking cold steel for mid 40'$.
My last one was from BudK and was very inexpensive but seems quite nicely put together.
By the time you reforge it, rehandle it, sharpen it. What is really left of the old hammer? Just the steel.
And I suspect it would cost you a pretty penny. Just my two cents, its your hammer and money.
