Does it come with head spaced bolt?

Sorry, no bolt included. Just what's in the picture.

Barrel was purchased in Nov. 2017. I just have too many other things I like shooting more.

I've included a picture of the muzzle crown and also one showing the locator pin on the extension and the gas port are in alignment. I've heard on older BCA barrels there were issues with them being offset.
