"Which model, which year, what brass brand, reloads? Factory loads?

I know my Win 94's both pre and post 64 have one round half out of the mag in "ready" position.

I also know that if you short stroke it, so the elevator does not bring the bullet up to be picked up you can very easily end up in the situation you describe.

Overall length can also be a factor with some models/calibers.
I am no expert, being new to the lever guns.
But the information is out there to be found.
But as always start by giving ALL the details.

As noted, more info is necessary to help you.

Yes, more needed. Does the loaded round go under the carrier?
I take it, it is a Marlin.
The lip on the bottom front of the carrier holds the next round in the magazine, might be something wrong there or for some reason the carrier is rising too far.
Use a strong flashlight and look into the loading port as you work the UNLOADED gun lever. You can see the action.

Are you able to make a video?

I had two Marlin lever guns, both had timing problems. One took a replacement lever, the other a replacement carrier.

Those parts wear a lot faster than you'd think, especially if they are run dry (grease is your friend).
