If I make a thick cylinder with a small hole in the base ie .200 and make a piston to suit,can I use it on a hydraulic press to make lead cores?

Yes you certainly can. That is how lead wire is made.
Be sure to give the hole a good lead, and a bit of taper to the exit. Make the piston fit close, a running fit will do fine.
The extruded lead can get hot if you move it quickly, so be careful handling the output. Heating the cylinder & base stock to about 100c will reduce the pressure required to extrude...

It is also a VERY GOOD IDEA to lubricte your lead. I used to buy .550 diameter lead wire that was "lubed" with some type of heavy oil.
