In that report we find;

At the heart of the tragedy was the fatal intersection in transference of risks from participant to observers. One moment, spectators were thrilled at the spectacle of speed only to have it followed by inescapable tragedy. The pilots understood the risks they assumed. The spectators assumed that their safety had been assessed.”

That is an interesting observation that could very well be applied to some reloaders who push the limits with reloads, both cast and jackted. Pushing the limits means not only to or above recommended psi levels but also with the improper use of some components.

I knew it was flutter the same day. You could see the missing tab in one of the pix,
and this is almost certain to be due to flutter.
