How to contact MP-molds for custom job?

I see that MP-molds has done group buys for folks here. I went looking at their website and all they have is a couple web-forms for contacting them. I filled out one form with an inquiry about placing an order but I haven't heard back. Do these folks have an email address or phone number or something?
