hello
sometime an spent case is not extract entirely of the chamber ,it come out about 1 inch and i must grab it with fingher .
must change the extractor or i can ajust by bending ,the hook is good

Look at the surface of the hook. Does it seem to be worn? If worn slightly, possibly
you can stone the hook back to a 90 degree angle so it will not slip off of the rim.

Some of the old Winchesters I have shot have had weak and worn extractors.
It has become a habit to hold down on the extractor with the left thumb on those.
If that don't do it it is fairly easy to replace it or resharpen the hook.
I have annealed and bent and re hardened and drawn the temper on them. Only takes about twenty minutes. The 92s are the worst offender.
A lot of times they just need a good cleaning. Crud gets underneath.
