I believe it has to be removed by unfastening it from the bottom of the sizer first.

A call to Lyman helped me figure it out. The problem was the piston had dropped below the threads on the pressure screw. After unmounting the unit, a light tap on the pressure screw drives it downward enough to re-engage the threads. Once the piston is out, the pressure screw drops out the bottom with a light tap.
Lyman said the recent units have a nylon sleeve that sits at the bottom of the well to prevent the piston from dropping below the threads.
Lyman is sending (free to me) a new pressure screw to replace the one I damaged trying to remove and a nylon sleeve.
Knowledgeable people manning the phones at Lyman and good customer service.
