My HDM has a safety screw with a flat head, not rounded on the edge like yours. Maybe yours is just a replacement screw that is too long. Since the diameter of the threaded portion and the head are apparently OK, you might try shortening it a tiny bit at a time until it will tighten the safety.
Is the detent ball still in the safety lever? If it is gone, even with screw tight, safety lever will be floppy.
The screw doesn’t seem to be hitting anything, the detent bal is in the underside of the safety lever it’s self and is still there.
When the screw is in with no lever on it does the screw sit flat against the frame?

I have a good HD-MIL and mine was doing that several years ago. I forget what it was but was a
simple fix. I'll look at it and post what the problem was. Last trouble I had was with back sight
moving while shooting. I put a shotgun shot ahead of the sight screw and that solved that problem
People don't realize that sight retaining screw is there. They drift the sight and the screw will not
lock the sight anymore. The OPs gun looks like someone has been into it more than once.
Let me know what you find out, I got it as is.
I wanted to bump this thread because I still have not figured this out
Looking at the schematic at gun parts corp it indicate the safety with a safety ball which you have. Is there a spring behind the safety ball. If you've determined that all parts are present I would take the cautious approach and buy or make shim washers to fit between the safety lever and the bolt that holds it in place.
If the schematic I'm looking at doesn't look like your gun ,then you need to find a schematic that does so anyone trying to help you will be on the same page.
