It's an anti aircraft round. They're likely using it as antitank/vehicle gun.
A rather crude iron sight, not a sniper rifle by a long shot (pun intended).
Syrian rebels stripped armaments from jet fighters and mounted these on trucks earlier on. One truck had a multiple rock pod from a old soviet era aircraft. Didn't look like it had any form of sighting or traversing equipment, they just aimed the whole truck at a target.
The Italians had bought a lot of lathi or solotrun 20mm anti tank rifles with built on scope sights before WW2. When they found these useless against allied tanks in North Africa they began using them as long range sniper rifles, scoring kills at up to 2,000 yards.
It has a sight system on par with a bead on a shotgun. That ain't no sniper gun. Close range car/truck killer as noted.
During WW1 U S troops used a lightweight man portable optically sighted 37mm cannon as a counter sniper weapon. They only had to get within ten yards or so of the target to put a sniper out of business.
20mm sniper rifles have been in limited use in some European armies and IIRC in South Africa, mainly as anti-material rifles.
One I remember has a pistol grip and trigger mounted directly on the massive bolt. The operator kept the bolt in a standard flap pistol holster till ready to use the rifle.

Talk about making your ears ring, WOW. The concution from the barrel being inside the room would be horrific. That sure is a big recoil spring on the front. I would not shoot it!

Wonder how ISIS big gun will fare against a smart bomb? Maybe that's why they took the pictures inside a building and not in the open air - they must be getting a little bit nervous of airstrikes by now.
Knowing what we have and what that weapon is effective against and how they are pretending to employ it, it's absolutely hilarious. I know the press is sadly ignorant, but ISIS attempted usage of this particular weapon as a hand aimed weapon is just silly.
A-10 Worthog would get the job done on that or the convoy.
