I would think hangfires, squibs, high shot to shot variation would be a sign. I often use dacron filler in my 375 H&H with 4759 powder. My definition of a filler, is a pea sized piece of dacron pushed down the case on top of the powder, expecting it to expand, and with careful handling, will keep the powder at the base of the case. I store and transport all my ammo, bullets up in the ammo case. I feel that the volume of the dacron is so small, it is consumed inside the case, and does not act as a secondary projectile. Using larger amounts of dacron is a different story, incurring greater risk of chamber ringing.
Universal is extremely position sensitive. And the 375 H&H case is very large. Unless you’re point the rifle straight up before each shot and then level there could be large velocity swings. Doesn’t seem this way from your testing so far. But I’d want to keep the powder against the primer.
I did point the rifle up before each shot to seat the powder against the primer.
Tomorrow I will experiment with that. Fed 215 might have helped too.

Back from the range. No dacron or filler. Tried the rounds from all positions, powder against primer, powder against bullet, powder flat on the bottom.
15.0 grains Universal, Fed 215 PC cast 274..........1344, 1329, 1336, 1332, 1342, 1345, 1337, 1339, 1335, 1340.
15.2 grains Universal, Fed 215 PC cast 274..........1351, 1348, 1359, 1350, 1343, 1347, 1358, 1349, 1355, 1345.
Just FYI
13.0 grains Red Dot, Fed 215, PC cast 274...........1251, 1247, 1251, 1244, 1253, 1250, 1239, 1259, 1251, 1248.
13.0 grains Red Dot, CCI 200, PC cast 274...........1246, 1234, 1242, 1244, 1242, 1246, 1246.
These are very mild loads in a 375. Most loads were 1" to 2" at 50 yards.

I think the 1240 on 15.2 Universal was an chrony anomaly. That shot did not feel any different and hit the same place the others did. Seems pretty consistent over 60 rounds.

Update.
Settled on 16.3 Universal. 1.5 inches at 100 yards.
Model 70 375 H and H Safari Express.
Fed 215.
Lyman 375449 Powder coated and sorted to 2 grain increments. Casts at .378ish #2 alloy. Sized to .377 after coating.
Brass mostly Hornady. Lee neck sized only. Loaded dozens of times so far. COL at 3.41. No crimp just straightened out slight bell.
Velocity very consistent around 1400 FPS. No filler or gas check.
Lead at $1.00/lb, 274 grain, 25 boolits/lb = .....0.04 cent/boolit.
Primers = .................................................. ..0.04 cent
Powder $189 for 8 lbs, ..................................0.054 cent
Brass = N/A

Easy on the shoulder. Easy on the wallet. Hunting accuracy at 100 yds with a big heavy large meplat boolit.

I just found a box of sized, lubed and checked 378-275-FN about an hour ago. Was under a bag of rags on my bench. There was even a bag of brass w/ it. Brass was tarnished so it’s being cleaned. Just need to get a scope for the rifle. It got pulled off and put on on of my hunting rifles since the 375 is just a range toy.
