If it's smaller than the barrel's groove diameter, you're screwed.

Depends. Best accuracy can come using either method depending on what problem it solves.

Cylinder throat is the best answer, but BA becomes absolutely correct when a soft boolit is bumped up to cylinder throat dimension. This happens more often than you would normally think. Typically accomplished when using fast powders with soft boolits, like +P+ loads (6 grains 700X, BE) in 45 colt (456 throats) with softer acp boolits (sized for acp, circa 452), for example. ... felix

Assuming good "range relationship"--i.e., alignment of throats with bore line--throat diameter should predicate boolit diameter. I developed what appears to be a very good situation with my Ruger Bisley Blackhawk in 45 Colt after reaming the .449" throats (!) to .4525", then polishing them to .453". I use .454" boolits through .453" throats into a .452" groove diameter. Accuracy is at least VERY good with every cast boolit I've tried in the revolver since that modification--200 grain SWC's, 230 RN, 250 SWC (both PB and GC) and 300 WFN/GC. To say I'm happy with the outcome is a large understatement.

I strive to size my boolits AT or .0005:-.001" over largest throat diameter--whether it's a revolver throat series or a rifle/pistol ballseat.
