Using a Lee carbide sizing die I was resizing my .38 Spec cases and one of the cases pushed the decapping pin up slightly. The next two cases the pin did not push the primer out of the case and they were stuck in the shell holder so had to remove the shell holder and use a separate decap rod to finish knocking out the primer.

I loosened the decapping pin on the die to see what was going on.

How I managed to hit that 22 case inside the 38 Special dead center and how it managed to stay in the 38 Spec case thru cleaning and being throw into a bucket I will never know.

Me too, with RCBS TC 38/357 sizer and a 22 LR hull. The handle stroke felt "funny", so I backed off and saw the pin retainer collar had a very belled 22 case mouth and a punch mark almost through the 22 case's base. I pulled it off with needle-nose pliers and went along with the processing.

Dito here only was loading 357 on the Lee 1000. Stroke felt kinda weird and the decapping pin popped up like yours. Had to pull the 22 case of with pliers reset the the decapper and got on with it.

I kept looking at that first picture and looking and looking.
I'm sitting here thinking that has to be the biggest de-capping pin set up ever, and how did he get a shot shell de-priming set-up in his 357 die.
then finally it was.
that case is an odd one there ain't a head stamp, and that primer pocket is totally,,,,,, oooooh,, ahh,,,, okay now I see it.
DOH!

Looks like you turned that 22 RF into a center fire case!! Now if you can just find a primer to work in it!

Guess if it can happen, eventually it will.

Interestingly enough I had it happen once, just about a month ago. It took me a minute to figure out just what to heck happened.
