What's a good phone app that I can record reloading data and shot placements on target? What do you guys use?

I'm not sure what you mean by "shot placements on target?" Do you mean group sizes. Or perhaps you're thinking of "come ups," as riflemen sometimes say, meaning adjustments for various ranges.

Any database or spreadsheet will suffice. A laptop computer may be more appropriate, as the volume of data can be large if good records are being kept.

Google Docs spreadsheet should suffice.

I scan my targets (after shooting) with a piece of dark green plastic (for contrast){ a piece of black construction paper would work well but didn't have any} and save them on my computer. I write on them which shot in sequence and date, where, who how far, and all the necessary stuff. I know that is not the answer to your question, just another idea.

If you want to scan your targets and convert them into high quality PDFs then get the program TurboScan. It uses your phones camera. The quality is really high contrary to what you might be thinking. I use it all the time to convert anything into PDFs. You can then link these PDFs to your spreadsheet.
